Abernethy malformation: case report and literature review

Congenital portosystemic shunts (CPSS) are rare congenital vascular anomalies associated with partial or complete diversion of the portal blood into systemic circulation. extrahepatic (CEPSS) termed Abernethy malformation. This pathology is a diagnostic challenge due to its low incidence and variable clinical presentations. We report case malformation Type Ib in 15-year-old male long-standing history high arterial pressure, recurrent nose bleeds, chest pain, dizziness, shortness breath, exercise tolerance, stool, vague epigastric nausea, itching. Imaging studies revealed dilated vein conduit flowing directly inferior vena cava (IVC), bypassing porta hepatis. Among other findings were multiple liver nodules, dilatation heart chambers, myocardial hypertrophy, pulmonary hypertension. Because severity patients symptoms, shunt anatomy, transplantation was recommended after multidisciplinary panel consultations. Diagnostic algorithm treatment options discussed as well.
